Changing the Name of a Registered Pharmacy

If you are applying for alterations or changing ownership of a pharmacy you can use those forms to change the registered name of your pharmacy at the same time. The Authority recommends all changes to banner groups (such as Chemist Warehouse, Advantage, Guardian, Terry White Chemmart, Amcal et al), be reflected in the pharmacy name.

There is no charge for processing the change of pharmacy name.

If you are not undertaking alterations or changing ownership but wish to change the pharmacy name, please complete Form PNC – Application for Change of Pharmacy Name

Once the application is processed, you will receive a new Premises Certificate to reflect the new pharmacy name.
